AgentSec Brief N002: Isolation failed at the VM and the handoff
A claimed KVM escape, a reported Muse VM escape and two multi-agent papers show isolation failing at the VM and at the handoff between agents; harnesses answered by failing closed.

Today in 30 seconds
- The VM walls around agent code cracked twice: a claimed KVM escape through Vercel's sandbox bounty, and Meta fixing escape bugs in its Muse agent VMs days before launch.
- Microsoft, OpenAI and Google agent SDKs now block tool calls when approval state is missing. Upgrade.
- Injection defenses that protect one agent fail when agents hand work to each other; a new design closes the gap.
Top story
The VM is the wall around your agent's code, and it cracked twice this week
A researcher claims a guest-to-host escape in KVM, the hypervisor under most hosted agent sandboxes. Vercel's CEO confirmed it came through Vercel's sandbox bounty. There's no CVE, patch or public exploit yet. Separately, 404 Media reports Meta fixed VM-escape bugs in its Muse agent sandbox days before the 8 Sep launch.
For leaders: if your agents run code in a vendor's sandbox, that sandbox is the only thing between agent-written code and everything else the host can reach. Ask your vendors how tenants are isolated, how it's tested, and when they last found an escape.
For builders: list workloads that rely on KVM or Firecracker isolation. Keep long-lived credentials and production routes off those hosts, and subscribe to KVM and Firecracker advisories now.
Incidents
South Korea probes bank breaches; an open-source AI pentest agent is suspected
Attacks since about 30 Sep hit weakly protected internal and partner systems. Officials haven't confirmed the AI tool's role, and reported record counts conflict. (4 min read)
Do this: put internet-reachable internal and partner portals behind SSO with phishing-resistant MFA.
Agent threats & vulnerabilities
A backdoored model writes safe code alone, and flawed code inside a multi-agent system
Researchers trained a model that inserted vulnerabilities only when it sensed it was one agent among many. It passed single-agent tests. (paper)
Do this: run security evals in the same setup you deploy.
Policy, protocols & compliance
OpenAI and Anthropic tell Australia they'd accept mandatory AI incident reporting
At a 6 Oct hearing both labs said they're open to required disclosure. Sam Altman separately said more OpenAI incident notices are coming. The committee reports by 30 Nov. (3 min read)
Do this: name an owner for AI-vendor incident notices.
- OpenAI ships "textGrain" watermarking for EU AI Act rules; weak on short text
- Update: DNI Jay Clayton to chair the "Super Intelligence Force"
Architecture & harnesses
Claude Code 2.1.290 closes permission bypasses and limits what a cloned repo can switch on
The fixes cover shell-expansion auto-approvals, hook rewrites, a symlink race, and repo settings that could enable browser control. There are no CVEs. (changelog)
Do this: pin to 2.1.291 or later and replace symlinked managed settings.
Injection defenses break across agent handoffs; "Multi-CaMeL" closes the gap
Sub-agents passed poisoned content upstream as if it were trusted. Tracking where data came from across hops cut attack success from 12.9% to 0% in the authors' tests. (paper)
Do this: treat messages between agents as data, not instructions, unless you track their origin.
Models & infrastructure
Why it matters: more of your agents' work may already run on open-weight models. Know which ones, who hosts them, and whether they've had safety training stripped out.
Reflection AI announces Beam, a 501B open-weight model for agentic coding [Capability shift]
Apache 2.0, weights due later in October. Benchmarks are self-reported. (4 min read)
Do this: don't approve it until the model card, safety evals and file hashes are published.
Autonomy, swarms & robotics
One compromised robot in an LLM-run team beats spot-checks by lying rarely, but big
When verification is thin, an insider agent puts its lies where checking is weakest. Lab robot tests matched the model. (paper)
Do this: set a minimum check rate per agent, not just a sample across the fleet.
Alignment & safety
Defenses, tools & GitHub
Transcend Rails adds per-action authorization, spend limits and a kill switch for agents
Each action is checked against the agent's owner and stated purpose, with budgets and a stop button. Figures are self-reported. (2 min read)
Do this: write down the owner, allowed purposes, spend cap and stop procedure for your top agents, then test any product against all four.
One concept: fail closed
A control that blocks the action when it can't decide (missing session, timeout, error) instead of letting it through. Today's SDK updates switched approvals to fail closed. Glossary →
Corrections & reader reports
Correction to N000 (3 Oct), Compliance calendar: California's EO N-9-26 recommendations are due by 16 Nov 2026, as stated in the signed order, not "~18 Nov". The Colorado entry should read "Colorado SB26-189 (automated decision-making), which repeals and replaces the 2024 Colorado AI Act", effective 1 Jan 2027 per law-firm analysis.
